To speed our investigation, you should include the following information and documentation with your letter:
- Full name, address, phone number, Social Security number, and government-issued photo identification.
- Written statement of alleged identity theft with as much specificity as you can provide. This statement must be signed and dated (sworn affidavit).
- Copy of an official, valid report filed with a federal, state, or local law enforcement agency, including the U.S. Postal Inspection Service. (police report or offense report)
- Any other information or documentation that is relevant to the alleged identity theft.
